If your domain is banned from appearing in the search index, it is quite easy to get it unbanned.
Submit your site to webmasters tools in Google (you must verify that you are the owner). Then use the "submit for re-inclusion" tool to re-submit your site. Make sure you address any possible reasons for Google banning your site. Give a very good reason for un-banning, and in a short period of time, Google may include your site in the search index. Note that what ever stuff Google indexed in your site still remains available.

*If your site is banned from Google AdSense, you cannot get it unbanned unless you can prove that you did not go against the TOS.
*These are 2 of the few services that concern websites that Google practise banning.

yes, maybe a letter to google, this maybe one thing that you can do.

and admit the wrong act and promise not to do it against the TOS.

maybe ok, or not (no approval)

if me, I will make more sites with adsense advs (255+ ) and tell them that I obey the TOS afterwards, never break it again.hope that they would see and understand. (we are human not robot, we may make some mistakes)

TOS is not allowed on self-clicking on the adsense advertisements or un-approved clickings on that advs.
since clickings related with $$$ payout.

yes, if no or less clicking at the adsense ads, very little profit can be earned.

unless the website is a large one with many traffics, such as site with a pr6 or above.

You don't realy get banned from Google in that you still can visit the sites even if you have been banned from a service.
Services that Google practise banning:
-AdSense [banning practise is fairly common - it is important to follow the TOS; Very difficult to get unbanned.]
-Google search [ban on sites that do not meet guidelines. Banning only means that the site would not be indexed. You can request for unban.]
-GMail [ban on email accounts that violate TOS. The account is "deleted", and cannot be registered again. The user however, can register a new account.]
-Blogger [rare. Google only bans users from a particular service upon court order.]
